What we look at
Consistency, completeness and calendar
The scan focuses on the three failure modes we see most often in institutional enrollment records.
- Whether entity names, addresses and identifiers agree across the information available.
- Whether ownership and management information appears complete and internally consistent.
- Whether any known revalidation or reporting window is approaching.
- Whether the provider types and locations on file match the operation you describe.
- Where a transaction or planned change is likely to create additional filings.
