Enrollment records drift. Facilities move, officers rotate, banking updates, a legal name gets restated — and one day PECOS says one thing, NPPES another, and the IRS letter a third. Then an application comes back, a claims edit fires, or a revalidation exposes the whole tangle at once.

Rejected and returned applications, diagnosed and refiled. Deactivated billing, addressed through reactivation filings with root-cause cleanup so the record does not fail the same way twice. And the quiet class of problems: records that no longer match reality — legal business name and EIN mismatches between IRS documentation and PECOS, NPPES entries that disagree with enrollment data, addresses, officials and ownership entries that lag the facts by years.
Why refiling alone fails.
The instinct after a rejection is to resubmit faster. But a filing generated from the same unverified facts fails the same way, one cycle later — with more elapsed time and less patience on the other side. Corrections that hold start with the record, not the form.
Reconcile, then refile.
Our sequence is deliberate. First, root cause: what the reviewing agency actually objected to, in writing, not what someone recalls. Second, reconciliation: we build or repair the Provider Master Record, comparing PECOS, NPPES, IRS documentation, state records and your source documents until every contradiction is logged and resolved with you — never silently. Third, order of operations: some corrections must land before others can file, and sequencing errors manufacture fresh rejections. Fourth, refiling under dual-control QA, through your own access, with your authorized official certifying. Then follow-up until the determination issues and the record reads clean end to end.

Not a resubmitted form — a record that agrees with itself, documented field by field, and a closeout file showing how each mismatch was resolved. Kept that way, if you choose, under ongoing maintenance or monitoring.
