Building a Provider Master Record

Most avoidable enrollment problems trace back to the same cause: two filings that describe the same organization differently.

Why one record

Enrollment filings are consistency tests. When an entity name, address, officer or identifier appears one way in a state filing and another way in a federal one, the difference gets noticed and the file goes into development.

What it holds

  • Legal entities, their relationships and their governing documents.
  • Owners and control interests, with the evidence behind each.
  • Officers, directors and managing employees, with effective dates.
  • Facilities, practice locations and mailing addresses.
  • Identifiers: NPIs, tax identification, license and certification numbers.
  • Program and payer enrollments, with status and known dates.

Maintenance is the point

Building the record once is a project. Keeping it accurate is a habit. The record only earns its value if it is updated as changes occur, because that is what makes the next transaction, survey or revalidation routine.
